Creating an online self assessment with results

Hi all,
I have used Sitely/Sparkle for some years now but I am a real novice and have little idea of website building as it was all mostly completed by web volunteers (for our registered charity).
I need to design a page for people to visit and answer each of the 10 questions on the form ticking a score or dropdown (1-10) thereby scoring each question out of 10.
Ideally, we need a total score for each section and a grand score total at the base.
So people visit, complete the self assessment, see the scores and can only receive the results if they complete the form to send us the results and their contact details so we can deliver a full report to them.
We also want to build in a Forum page for points and discussions. (Does Sitely have a Forum that can be inserted)?
I appreciate that this is a lot to ask, but any help would be very warmly welcomed.

If I understand correctly what you want to achieve with your form, it is doable in Sitely with the help of calculators, but it requires some knowledge of JavaScript. Natively, without adding JavaScript code, this won’t be possible.

Regarding the forum, Sitely does not offer this functionality. You will need to use external services.

Maybe have a rethink @Mac in how you want to collect the data to send out the report email? You can collect the needed data on the one page with collection container for name, email, etc… It will be a bit more manual as you receive the email with the information that you need and then you’d sort it out on your end before sending out the report.

Regarding the forum. The best suggestion would be a third-party forum that you can embed into Sitely giving the User a seamless experience. Something like - Forum Widget - embed a community widget to your website! (2024)
